Signature Care - Services & Pricelist

Initial Visit & General Care

Initial Consultation – $100 per visit

Required for all new clients. Includes medical history review, medication review, vitals, home safety assessment, and individualized care planning.

Additional services at intake may incur extra charges to be discussed before a visit.

Add-On Fees:

  • Non-urgent same-day visit – $50 per visit
  • Urgent same-day visit – $100 per visit
  • Distance fee (25–50 miles) – $50 per visitSame-day and distance fees are separate from the cost of services.

General Nursing Services

  • Phone triage or lab/imaging review – $30 per visit
  • Medication administration or organization – $65 per visit
  • IV antibiotic administration or TPN – $75 per visit
  • IV hydration – $75 per visit
  • Tube feeding setup and education – $65 per visit
  • Tracheostomy care – $75 per visit
  • Foley catheter change (with order) – $100 per visit
  • Colostomy / ileostomy / urostomy care – $65 per visit

Wound Care Services

Applied when wound orders are already in place. If the NP must create the wound care plan, see NP Care Coordination section.

Pressure Ulcers:

  • Stage I–II or Deep Tissue Injury – $75 per visit
  • Stage III – $100 per visit
  • Stage IV – $150 per visit

Surgical Incisions:

  • Uncomplicated incision care – $75 per visit
  • Complicated incision care (may require NP evaluation) – $150 per visit

Other Chronic Wounds:
(Diabetic ulcers, venous stasis ulcers, chronic non-healing wounds)

  • $75–$150 per visit, depending on severity

Nurse Practitioner Care Coordination & Orders

(These are administrative/clinical order fees — not per-visit services.)

  • Wound care orders – $150
  • Durable Medical Equipment (DME) orders – $100
  • Home Health or Home Care orders – $150
  • Specialist referrals – $75
  • Medication refill (non-urgent) – $100
  • Medication refill (urgent) – $150
  • Letters, forms, FMLA, or medical documentation – $75

 

Urgent Care Services (Non-Emergency Only)

May qualify for insurance billing through the collaborating physician.

Signature Care is NOT an emergency medical provider.

Urgent Care Visit Pricing:

  • Base urgent care evaluation (for uninsured patients) – $150 per visit

This base fee does not include additional tests, treatments, medications, or procedures.

Services include:

  • UTI evaluation and treatment
  • IV hydration for mild dehydration
  • Mild cellulitis evaluation & antibiotic management
  • Upper respiratory infections, mild/moderate bronchitis, mild pneumonia (when appropriate)
  • Gastroenteritis (nausea, vomiting, diarrhea) with symptomatic treatment and hydration planning
  • Laceration repair (sutures, adhesive, or wound strips)
  • Rapid tests as indicated: flu, strep, COVID
  • Ear irrigation
  • Simple skin conditions: dermatitis, localized rashes, hives (non-anaphylactic)
  • Conjunctivitis (non-severe)
  • Minor musculoskeletal strains/sprains (RICE, basic support, referral if imaging needed)
  • Small abscess evaluation & incision & drainage when appropriate
  • Migraine/acute headache protocols (non–red flag) with hydration and medication support

(Procedure fees or supplies may apply depending on the treatment required.)

Urgent Care Follow-Up Requirement

All urgent care visits require a telemedicine follow-up with
the collaborating physician for safety and continuity.
This visit is billed separately and may be covered by
insurance.

Urgent Care Emergency Exclusions

Signature Care does NOT treat:

  • Chest pain
  • Shortness of breath or respiratory distress
  • Stroke symptoms
  • Heavy bleeding
  • Loss of consciousness
  • Severe allergic reactions
  • Major trauma

Clients experiencing these must call 911 immediately.

Important Things You Should Know

Signature Care is a private-pay, mobile nursing and nurse practitioner (NP) service providing high-touch, in-home medical care, wellness monitoring, chronic condition support, post-operative care, and limited urgent care services.
We come directly to your home for high-quality, personalized care.

Signature Care is primarily private pay.
However:

  • Urgent Care visits may be eligible for insurance coverage.
  • A collaborating physician may bill your insurance for telemedicine urgent-care follow-up.
  • All concierge nursing, wellness, and routine services are private pay only. We will review coverage during your intake visit.

No. Signature Care is not a primary care provider and does not replace your PCP.

We strongly encourage all clients to maintain a PCP for:

  • Annual physical exams
  • Preventive screenings (mammograms, Pap smears, prostate exams, colonoscopies)
  • Chronic condition management
  • Long-term medication renewals

We work with your PCP to support your health at home.

Distance fees apply for visits 25–50 miles from central Detroit.

  • Rapid tests: flu, strep, COVID

  • Ear irrigation for wax buildup

  • Conjunctivitis (pink eye) treatment

  • Skin conditions: dermatitis, rashes, hives

  • Minor sprains/strains

  • Small abscess evaluation & drainage

  • Migraine/headache relief protocol

  • Phlebotomy for urgent labs (CBC, BMP, etc.)

(Emergency Exclusions)

We do not treat:

  • Chest pain
  • Stroke symptoms
  • Severe breathing difficulty
  • Active heavy bleeding
  • Major trauma
  • Severe allergic reactions (anaphylaxis)
  • Loss of consciousness
  • Psychiatric emergencies

Call 911 or visit the nearest emergency department for these symptoms.

Yes. All clients must complete a $100 intake assessment which includes:

  • Medical history review
  • Medication list review
  • Home safety assessment
  • Care plan creation
  • Service recommendations
  • Coordination with your PCP if needed

This ensures safe, appropriate services.

Same-day visits are available when schedule permits.
Add-on fees:

  • Non-urgent same-day: +$50
  • Urgent same-day: +$100

Urgent visits may require a collaborating physician telemedicine visit (billed separately).

All concierge services are private pay and billed at the time of appointment.

Yes.

Our Nurse Practitioner can:

All prescribing follows Michigan NP regulations with collaborating physician oversight.

Yes.

At this time, we primarily serve adults and seniors, but may accept older teens for certain services.

With your consent, we can:

  • Update your PCP
  • Consult specialists
  • Share lab results
  • Coordinate home health referrals
  • Communicate care plans

We serve as an extension of your medical team.

We can help.

  • The NP can perform an assessment and create wound care orders ($150).
  • Once the care plan is in place, routine visits follow the standard wound-care pricing.
